Request a Certificate of Insurance (COI)

Complete the certificate request form. Existing clients can provide their insured business name, the certificate holder's exact name and address, recipient email, project details, deadline, and additional-insured or other insurance requirements in one place.

A certificate provides evidence of coverage currently in force. It does not add or change coverage or confer rights beyond the policy and applicable endorsements. Requests may need insurer approval. Call 903-372-1901 about an urgent deadline; a submitted request does not confirm issuance.

Request an insurance ID card

Email an ID-card request with your insured name, vehicle year/make/model, and preferred delivery contact, or call the office. Do not include payment details or Social Security numbers. You can also check your insurer's official account portal.

Request a policy change or renewal review

Email a policy-change or renewal-review request. Describe what changed and the requested effective date. We will follow up for any documents or sensitive information needed. Coverage changes take effect only after confirmation from the agency or insurer; sending a request does not bind, change, or cancel coverage.

Claims and billing help

Open carrier claim contacts to report a loss or find the insurer's claims site. For billing, use the phone number or official payment portal shown on your bill. Call us if you are unsure which insurer or account applies. Do not email payment-card or bank details.
